Is the questionnaire that the patient's parent fills out (for a child) considered part of the medical records? Or is it just the doctor's report?

NROTC Marine Option Shoulder Surgery Waiver
I am applying for the NROTC MO scholarship and I plan on earning a commission as a Marine officer after I graduate. Only problem is that I have dislocated my shoulder about 4 times and my doctors are telling me that I need surgery to stabilize it. THERE ARE NO TEARS IN THE MUSCLES OR LIGAMENTS, the surgery is just to tighten the ligaments surrounding it. I know that having a shoulder operation can potentially disqualify you from service. Do any of you know if they grant waivers often for...

Poison Ivy reaction?
Alright so I went to the doctor today (probably my first mistake) because I had this rash on my skin that followed me after I cut down a tree covered in poison ivy. Doctor called it "rhus dermatitis" and prescribed 10 mg tablets of prednisone (second mistake). Started freaking out because I saw that eczema and atopic dermatitis are DQ-able for AFROTC. I'm an AS200 and on scholarship so I already had my physical done but do I still have to report it even though I haven't taken the...

Request for waiver question
Has there been anyone who was denied from a SA because of a waiver that hasn't been approved by the April 15th deadline? I think I'm going to be on this boat soon b/c my requested waiver has not been granted yet and all I can do is panic.

Unexplained Syncopal Episode
My 16 year old son just received the DQ letter from Dodmers due to unexplained syncopal ( fainting) episode which occurred 2 years ago. We submitted EKG and Catscan results from the ER from that ER visit. Testing results were normal. The ER Dr thought he was fainting posibble due to dehydration due to a severe cold / Strep the day before. But he did not put it in writing....? Is there anything that we can do? His ultimate dream is crashing as we speak....waiting for a waiver from USMMA. We...

Do MEPS and DoDMERB cross reference records?

MEPS and DoDMERB
I'm sure you are all fed up with these posts, but my recruiters are clueless (as expected). When you go to MEPS I know you need documentation for a lot of things, I have had a concussion in the past (no loss of consciousness) and a piece of bone pulled off my finger by a ligament back in middle school among other issues. Am I going to need documentation for all of these things when I go to do my DoDMERB physical or do I just record it on my medical history form?

Asthma
I know that the medical regulations are that any history of asthma after age 13 is disqualifying, but I was wondering if there are any exceptions to that. I have exercise-induced asthma, and I use my inhaler before any rigorous workouts (I am 16). I still stay active and do sports, and I am very, very strong academically. Would there be any chance of getting a waiver for something like this, or is it hopeless? Thanks in advance.

Please clarify DoDMERB status....
I've read so much that I can't keep it straight between the 2 portals. Does this mean DS IS being considered for a waiver? Or just that he made it through the remedials and maybe USAFA isn't even looking at him because they only consider waivers, at this point, for candidates that they are very interested in?
DoDMERB says :
Current Medical Status: Pending Waiver Submission/Review
Code Description
R250.40 Pending waiver review by USAFA

Responding to Remedial (Additional Medial Information)
Hello everyone,
Apparently the physician forgot to upload one page from my medical packet, and I received this:
"Code: R251.02 Incomplete or illegible DD Form 2492 missing page 1 (front side) of the DD Form 2492."
The official letter says that I can either send it by physical mail or email it to usaf.dodmerb.helpdesk@mail.mil.
Can I just email it? The letter states explicitly that this missing page has to be mailed by physical mail, and only in the very end it mentions...

ICL wavier
My ICL wavier was submitted to cadet command on July 11 of 2014 for Green to Gold and I am still waiting for a yes or no. I was told that my wavier was sent to a specialty surgeon to get approval. I was wondering if anyone had this issue before or if they knew some kind of time frame. It is also getting to the time where I need to turn in my packet or I wont be able to start this January.

Disqualified - medical waiver denied
Hi,
My daughter was disqualified for the Air Force Academy due to 3 reasons.
1) History of systemic allergic reaction to shellfish/sesame.
2) History of asthma requiring medication within the last year.
3) History of osteochondral defect managed with microfracture surgery and foreign body removal.
We just received the letter from the Air Force Academy denying a medical waiver.
In 2003, my the 8yr old daughter had an allergic reaction to whatever she was eating while we were on a...

Elective cosmetic surgery after receiving medically qualified status
I am a winner of a 3 year AROTC scholarship. I was planning on getting gynecomastia surgery. It should be really simple a small incision below the nipple and they remove the enlarged glands. Recovery time should be only 2 weeks. What sort of action is required on my part with DoDMERB. I don't even think this would require me to check any marks on the medical form I had to fill out unless it was the other box.

Multiple applications, TWE's & DoDMERB?
What happens now? DS#2 was waiting for a waiver from USAFA until yesterday when he received his TWE. He is also waiting for a waiver from AROTC, for his plan b . Who would he contact to see where they are with his waiver? He is still in "pending waiver review" . We want to stay on top of it so he doesn't have to go to plan c.

Medically Disqualified from AFROTC, want to do OTS
I was an AFROTC cadet for 3 semesters, I had to take my medical test my 200 year and I told the absolute truth which was that I was taking medicine to manage anxiety (or better known, anti-depressants). I was then disqualified a week before the end of the semester, my cadre used every waiver and called in every favor they could. I didn't know that telling the truth about that medication would get me MD'd. I am not depressed, never have been and the only reason I was taking the anxiety...

Waiver for height and weight standards
I'm in 10th grade now, but I already know that I really want to apply to the Naval Academy.I was just looking at the disqualifying medical conditions here: https://www.usna.edu/Admissions/_files/documents/MedicalAppendix1.pdf and it said that since I am 5'7, I would need to weigh 121 lbs.
I've been 5'7'' and 110 lbs for a long time, and while I am going to try and gain some muscle in the next year, I am worried that I might not reach the minimum weight quickly enough. I'm in...
