How to schedule a remedial

DoDMERB sent me a letter yesterday stating that I need a medical AMI to be completed. However, Concorde/DODMETS has no option for me to make a new appointment. How would I schedule a remedial with them?

And if I went to a MTF, how would the paperwork be sent in?
