Hello, I have a question concerning the insect allergy remedial questionnaire. The form states to confirm if I have ever been diagnosed with an insect allergy. The problem is that I have never been to a doctor for my allergy; but rather, I diagnosed myself with the ant allergy because I do get hives and swelling at the bite.
