I am about to take my optometric exam for the DoDMERB and have my medical exam a few days after. However, I have not yet finished my "Medical History" forms. Do I need to have these completed before I go in to see the eye doctor? Are there any papers I need to bring with me to the optometric exam? I wear glasses but otherwise I've never had any problems with my eyes or vision. Thanks in advance.
