Hey guys I have my DODMERB physical soon and I just wanted to be sure of what to bring to the appointment. I know I need to bring my printed and completed packet (I had to redo my entire packet by hand because none of the questionnaire or signatures were registered) to my medical provider but what about my optometrist? Do I have to print an extra form to bring to them? Also my audiologist is in the same place as my medical provider so how will that work? I'm assuming I will have both audio...
