DoDMERB Followup Occurances: Professional Counseling Clarification

As part of the followup for the DoDMERB survey, they ask how many times a specific occurrence, in my case counseling, has occurred. When it asks for incidences, would it want how many times you went to a therapist (number of appointments) or how many times something has triggered you to go to a therapist (eg. moving or loss of parent forces you to go to therapy sessions)?

Additionally, it states they would need sealed medical information that would support what therapy you went to. Do...
