DODMERB Authorization page

Hello, I am new to all this military stuff and forums. My son just finished his DODMERB, but there was one form that was for authorization to discuss medical information to other parties. If he is not 18 yet, does he need to fill this out for medical professionals to talk to his parents? Does this give DODMERB people permission to talk with his family physician, orthodontist, dentist, etc?

I guess my main question is- should he fill this out and who should be put on the form? It asks...
